Job summary

Hartford HealthCare is seeking a part-time Registered Nurse (RN) for the Acute Rehab unit at St. Vincent’s Medical Center in Bridgeport, CT. You will implement care plans, monitor patient conditions, coordinate interdisciplinary care, and support discharge planning to ensure seamless patient recovery.

The role requires an Associate degree (BSN preferred), CT RN license, and BLS certification. Acute care experience is highly valued, with benefits including tuition reimbursement and nurse

Responsibilities

The Registered Nurse (RN) in Acute Rehab provides direct nursing care in accordance with established policies and protocols. This role is ideal for compassionate nurses who are committed to delivering individualized patient care.

What You'll Do

• Implement and monitor patient care plans.
• Monitor, record, and communicate patient condition as appropriate.
• Serve as a primary coordinator of all disciplines for well-coordinated patient care.
• Note and carry out physician and nursing orders.
• Assess and coordinate patient's discharge planning needs with the healthcare team.
• Demonstrate knowledge and skills to assess patients' physical, psychosocial, cultural, spiritual, and age-related needs.
• Delegate aspects of care to other nursing personnel consistent with their education and expertise.
• Participate in daily interdisciplinary patient rounds.
• Develop, implement, and revise individualized plans of care.
• Participate in activities for professional growth and development outside of patient care.

Requirements

**Education:**

• Associate degree required; BSN preferred.

**Experience:**

• Acute care experience highly preferred.

**Licenses & Certifications:**

• Current Connecticut RN license.
• BLS certification by the American Heart Association.
