Job summary

Grand Lake Health System is seeking a registered nurse for a full-time, 1st shift position delivering care in outpatient physician practices. You will assess patients, plan and implement care, and document in the electronic medical record.

Requirements: graduate from an accredited school of nursing, Ohio license, and BCLS certification. Outpatient clinic experience is preferred. Emphasizes patient privacy, dignity and safety.

Full Time, 1st Shift 80 hours per pay period No holidays or weekends required.

Duties and Key Responsibilities

• This is a float position responsible for delivering patient care in all Physician Practices through the nursing process of assessment, planning, implementation, and evaluation.
• Greets and escorts patients through the medical practice.
• Completes nursing assessments of patient by completing history of present disease and illnesses with accuracy and documents appropriately in patient electronic medical record.
• Collaborates with physicians and other health team members in implementing physician orders.
• Incorporates holistic approach to Patient Centered Medical Home, thus moving beyond problem focused care.
• Implements and documents clinical and technical aspects of care.
• Evaluates patient’s response and understanding to plan of care by using teach back method.
• Openly and appropriately communicates with patient, physicians and other members of health team.
• Serves as a liaison for patient with outside agencies, businesses and providers, establishing a therapeutic relationship with the patient and family.
• Respect patient’s rights to privacy, dignity and confidentiality.
• Maintains positive interpersonal relationships with all internal and external customers.
• Maintains established health system and work area policies and procedures, objectives, guest relations, safety, environmental and infection control guidelines.
• Performs other duties as assigned.

Requirements

• Clinical skills in delivering patient care in an outpatient setting.
• Obtains and maintains successful completion of BCLS Healthcare Provider Course.
• Communications effectively with physicians, patients and coworkers to maintain respect and a positive work environment.

Education/Certifications

• Graduate of an accredited school of nursing.
• Licensed to practice professional nursing in the State of Ohio.

Experience

• Previous experience in medical practice or outpatient clinic preferred.
