## Grand Lake Physician Practice Licensed Practical Nurse (Casual)Apply: Miami Erie Family Practice & Pediatrics: Casual: Posted 11 Days Ago: R-10037## **Hours of Job*** ## Casual employment working as needed based on departmental needs.* ## Pool Staff hours are Monday - Friday.* ## 1st shift hours. No weekends or holidays required.## **Duties and Key Responsibilities*** ## Greets and escorts patients through the medical practice.* ## Completes nursing assessments of patient by completing history of present disease and illnesses with accuracy and documents appropriately in patient electronic medical record.* ## Collaborates with physicians and other health team members in implementing physician orders.* ## Incorporates holistic approach to Patient Centered Medical Home, thus moving beyond problem focused care.* ## Implements and documents clinical and technical aspects of care.* ## Evaluates patient’s response and understanding to plan of care by using teach back method.* ## Openly and appropriately communicates with patient, physicians and other members of health team.* ## Serves as a liaison for patient with outside agencies, businesses and providers, establishing a therapeutic relationship with the patient and family.* ## Respect patient’s rights to privacy, dignity and confidentiality.* ## Maintains positive interpersonal relationships with all internal and external customers.* ## Maintains established health system and work area policies and procedures, objectives, guest relations, safety, environmental and infection control guidelines* ## Performs other duties as assigned.## **Requirements*** ## Clinical skills in delivering patient care in an outpatient setting.* ## Obtains and maintains successful completion of BCLS Healthcare Provider Course.* ## Communications effectively with physicians, patients and coworkers to maintain respect and a positive work environment.## **Education/Certifications*** ## Graduate of an accredited school of nursing.* ## Licensed to practice professional nursing in the State of Ohio## **Experience*** ## Previous experience in medical practice or outpatient clinic preferred.
