Gilead Community Services, Inc. seeks a caring LPN for its New Britain area adult residential and outpatient addiction program.

This per diem role involves providing medical care, supervising medication administration, and coordinating care with the treatment team and external providers, with shifts mainly evenings 4:00 PM–12:00 PM and weekends. The LPN will contribute medical knowledge to the team, conduct health histories and evaluations, and support timely documentation while upholding

Qualifications

• Licensed LPN in Connecticut
• High school diploma or equivalent; Associate/Bachelor preferred
• Experience with substance use disorders and psychiatric issues
• Five years of clinical experience
• Valid driver’s license and automobile insurance in good standing; reliable use of personal vehicle

Responsibilities

• Providing clinical knowledge regarding mental health and substance abuse disorders and medical issues
• Supporting the Program Director in leading the program through clinical services and best practices
• Coordinating with MDs and external providers for medical/psychiatric care
• Working with pharmacies to ensure proper pharmaceutical care
• Providing guidance and peer-support regarding medical care
• Conducting Health Histories, Health Assessments, and medical evaluations
• Making community referrals based on assessments
• Providing medical education to case managers
• Ensuring orientation and management of medication self-administration
• Completing all required documentation timely
• Acting with compassion, integrity, and ethics

Job description

Gilead Community Services is seeking a caring and recovery-oriented LPN to join our multi-disciplinary residential and outpatient addiction program.

About the position:

The LPN for Gilead’s New Britain area adult residential substance use disorder treatment program will provide medical care, oversee medication administration and pharmacology issues, facilitate coordination of care between the treatment team and external providers, and provide medical knowledge and education to team members. This is a per diem role, available shifts are primarily evenings, 4:00 PM - 12:00 PM and weekends.

Minimum Requirements:

• Must be a licensed LPN in Connecticut (CT).
• High school diploma or equivalent, Associate’s or Bachelor’s preferred
• Experience working with individuals with substance use disorders and psychiatric issues
• Five years of clinical experience
• Must maintain and provide proof of valid driver’s license and automobile insurance in good standing.
• Reliable use of personal vehicle as needed

Responsibilities:

• Providing clinical knowledge regarding severe and prolonged mental health and substance abuse disorders as well as medical knowledge related to health and well-being
• Supporting the Program Director in leading an effective program through clinical services and best practice models of care
• Maintaining contact with MDs (i.e. psychiatrists, PCPs, specialists, etc.) to coordinate medical and psychiatric care of individuals
• Working closely with pharmacies to facilitate proper pharmaceutical care
• Providing guidance and peer-support regarding medical care in their respective program
• Conducting Health Histories, Health Assessments, and medical evaluations of clients’ emergent medical issues
• Making appropriate community referrals based on assessments and evaluations to meet clients’ basic health needs
• Providing ongoing medical education to case managers informally and in training formats
• Ensuring proper orientation, training, and management of the medication self-administration system
• Completing all required documentation electronically and on paper in a timely manner
• Acting with compassion, integrity, and ethics

This is a non-union position, the rate of pay is $32.50 per hour.
