Job summary

Geisinger Wyoming Valley is seeking a Licensed Practical Nurse to serve as a patient navigator, guiding incoming calls and entering new patients into the program. The role provides patient education, supports understanding of the condition, and ensures continuity of care across clinic and didactic settings.

As part of a collaborative team, you will assist providers, document patient needs, and coordinate activities such as education series and support groups, while maintaining timely follow-up

Qualifications

• Graduate from Specialty Training Program—Nursing (Required).
• Minimum of 2 years nursing experience (Required).
• Licensed Practical Nurse license required.

Responsibilities

• Facilitate patient introduction to the program and provide education.
• Coordinate care with providers and follow up via telephone and documentation.
• Serve as patient navigator guiding incoming calls and entrance of new patients.
• Maintain records and ensure timely follow-up.
