Qualifications

• Licensure: PharmD and MI license required
• 3+ years of clinical pharmacist experience required
• GI specialty experience preferred
• Completion of PGY1/PGY2 residency preferred

Responsibilities

• Provide medication therapy management for GI patients, including reconciliation and adherence counseling
• Collaborate with interdisciplinary care team to develop GI care plans
• Monitor and evaluate regimens for therapeutic goals and interactions
• Identify opportunities for cost savings and optimize therapy
• Assess infusion and injectable GI therapies and monitor response
