Job summary

Oak Street Health is seeking a Primary Care Physician in Connecticut (New Britain) for full-time, on-site work at our neighborhood primary care centers. The role emphasizes equitable, value-based care for local Medicare populations, supported by large care teams to improve outcomes.

Responsibilities include assessing and diagnosing patients, overseeing primary care delivery, and managing medications within a collaborative leadership structure.

Qualifications

• MD or DO degree required and board certified or board eligible in Internal Medicine or Family Medicine.
• Active, non-probationary State License and DEA registration required.
• US work authorization is mandatory for employment eligibility.
• Fellowship in geriatrics is preferred but not required.
• Experience in geriatrics and value-based care is advantageous.

Responsibilities

• Assess and diagnose patients at local clinics.
• Oversee, direct, and administer primary care services.
• Prescribe and manage medications for patients.
• Maintain and update Patient EMR data in Canopy and Greenway.
• Collaborate with regional and central leadership to meet quality goals.
• Work with Practice Managers to lead the center care team.
