Job summary

Jobtailor is seeking a Panel Coordinator/Care Coordination specialist to support geriatric patients. You will work to manage patient panels, coordinate care, and outreach to improve outcomes.

The role involves hybrid work between clinical practices and remote assignments, with emphasis on EMR systems and MyChart for patient engagement.

Qualifications

• High School Diploma or GED.
• Completion of an accredited health program (e.g., Panel Coordinator Onboarding, Medical Assistant or CNA).
• 3+ years in panel management, care coordination, or similar role.
• Bachelor’s or associate degree in a health-related field preferred.
• Hybrid work between clinical practices and remote/home.

Responsibilities

• Support geriatric providers through proactive panel management and patient outreach.
• Monitor chronic disease registries, preventive care gaps, and payer reports.
• Conduct outreach via MyChart messages, phone calls, letters, and other communication methods.
• Partner with Nurse Navigators and interdisciplinary care team members to coordinate services.
• Schedule appointments, tests, and follow-up services based on patient needs.
• Assist with onboarding new patients and distributing welcome materials.
• Support population health initiatives, performance improvement programs, and quality measures.
• Review EMR and payer data to validate care gaps and improve outcomes.
• Educate patients and families on preventive care, chronic disease management, and care plan adherence.

• Support population health initiatives, performance improvement programs, and quality incentive measures
• Review EMR and payer data to validate care gaps, support accurate documentation, and improve outcomes
• Educate patients and families on preventive care, chronic disease management, and care plan adherence

Requirements

• High School Diploma or GED
• Successful completion of an accredited/approved health program, such as IHA’s Panel Coordinator Onboarding Program, Medical Assistant program, Clinical Support Assistant, Certified Nursing Assistant, Patient Care Technologist, or another clinical health certificate program approved by Quality Department Leadership
• 3 or more years’ previous experience in panel management, care coordination, or a similar role
• Strong preference for prior Panel Coordinator and/or Medical Assistant background
• Experience working with geriatric population and/or chronic disease management preferred
• Bachelor’s or associate degree in a health-related field or relevant experience strongly preferred
• Ability to work hybrid between support practices and remotely from home
