Job description

Job Responsibilities:

1. Provide primary medical coverage in collaboration with PO physicians as needed. Perform initial, semi-annual, and annual assessments; conduct additional assessments as needed; and implement appropriate care plans for PO participants.
2. Participate in discharge planning for both acute and long-term placements.
3. Evaluate and treat participants during acute illnesses. Prescribe medications, therapies, and treatments prudently. Refer participants to specialists following consultation with the Primary Care Physician.
4. Make nursing home or home visits when necessary.
5. Collaborate with the Medical Director and Clinic Supervisor to develop and monitor clinical policies, procedures, and standards of care. Provide guidance and training to staff on medical and quality assurance issues.
6. Advise on accountability improvements for PO services and act as a resource during participant intake and daily operations.

Requirements:

• Five (5) years of experience, preferably in long-term care.
• Active PA License.
• Board Certification by ANCC or AANP.
