Seeking a Nurse Practitioner to deliver effective and equitable value-based primary care to Medicare patients in Reading, PA.

Opportunity Details

• Bilingual Spanish preferred
• Full-time, Monday to Friday, 8am - 5pm
• Master of Science in Nursing, Doctor of Nursing Practice, or NP Certificate Program completion required
• National certification as a Family, Adult, or Adult-Gerontology Primary Care Nurse Practitioner through AANP or ANCC
• Experience in primary care settings such as internal medicine, geriatrics, or family medicine preferred
• Knowledge of HCC documentation, ICD-10 coding, and Health Risk Assessments
