Position Summary:

Eventus WholeHealth delivers an integrated model of care to adults residing in skilled nursing and assisted living facilities through a network of healthcare providers including Physicians, Nurse Practitioners, Physician Assistants, Medical Assistants, and in-house Support Staff.

Essential Duties and Responsibilities:
  1. Provide primary-care services in accordance with site-specific protocols and the standard of care for family medicine, geriatric medicine, and/or internal medicine related to long-term care facilities. This includes reviewing and signing MARs, managing orders, addressing labs within 24 hours, managing narcotic prescriptions, and completing administrative paperwork efficiently.
  2. Complete documentation and submit billing records as per policies, providing a hard copy of documentation within 48 hours for patient records.
  3. Maintain open communication with facility staff and administration, providing verbal and written instructions and feedback, and coordinating with facility care coordinators daily.
  4. Adhere to Eventus WholeHealth standards, policies, procedures, and all legal, regulatory, and ethical guidelines, including HIPAA compliance and confidentiality.
  5. Consult with the clinical supervisor before refusing resident, family, or facility requests regarding resident care.
Qualifications/Education:
  1. Master's degree in Nursing from an accredited institution with a current, valid state license; or
  2. Graduation from an accredited Physician Assistant program, passing the national certification exam, and holding a valid state license; specialization in geriatrics, family, or psychiatry is preferred.
