Shift: Day Job

Hours: Monday - Friday 8:00 AM - 5:00 PM

Make a Difference

This role offers the opportunity to impact the lives of patients with complex or chronic conditions. The position is primarily remote, with interactions occurring via phone, in-home, in-office, or hospital settings, tailored to patient needs.

Qualifications and Skills
  • Graduated from an accredited nursing program (ACEN or CCNE)
  • Licensed Registered Nurse in Indiana
  • Minimum 3 years of nursing experience, preferably in population health, home health, ambulatory care, or case management
  • Experience with electronic health records; EPIC experience preferred
  • Knowledge of self-management education; experience with Population Health Teams or ACOs preferred
