CareConnectMD, Inc. is seeking a compassionate Nurse Practitioner to deliver patient-centered primary care to older adults in assisted living facilities in the San Diego area. The role emphasizes preventive care, chronic disease management, and close collaboration with facility staff and the care team.

Ideal candidates will have a Master’s degree in Nursing, board certification as an adult/geriatric NP, and at least three years of geriatrics experience. Local candidates are preferred.

Qualifications

• Master's degree in nursing from an accredited program.
• State board certification as adult/geriatric nurse practitioner.
• Minimum 3 years of geriatrics experience; long-term care preferred.

Responsabilités

• Provide primary care in assisted living facilities for older adults.
• Coordinate with interdisciplinary care team and facility staff.
• Manage chronic conditions and preventive care plans.
• Document medical records timely within facility policies.
• Educate patients and families about disease management and wellness.
