Job summary

TruHealth is seeking an advanced practice provider, either a nurse practitioner (NP) or physician assistant (PA) with experience in primary care or internal medicine, preferably with geriatrics focus. This position gives you the opportunity to improve outcomes for long-term care residents by collaborating with a team of providers and case managers.

Qualifications

• Experience in geriatric nursing or rehab within long-term care.
• Degree from an accredited APRN or PA program.
• Masters or higher required for this role.

Responsibilities

• Assess patients’ medical and mental health needs and plan proactive care.
• Prescribe medications and order labs, diagnostics and consultations.
• Monitor treatment responses and adjust plans; coordinate with RN case managers.
