Pine Park Health is seeking a Full-Time Nurse Practitioner to deliver onsite primary care to seniors across Palo Alto, Redwood City, San Mateo and surrounding Peninsula communities. You will work with a multidisciplinary team to create detailed care plans and support a value-based care model.

This role requires a compassionate clinician who thrives in a changing environment and is willing to travel between partner communities to improve patient outcomes. Hybrid work arrangements may be offered.

Qualifications

• Must hold an active Nurse Practitioner license.
• Geriatrics experience preferred.
• Comfortable in fast-paced environments and travel within the San Francisco Peninsula region.

Job description

Pine Park Health is looking for a Full-Time Nurse Practitioner responsible for delivering care in our San Francisco Peninsula market (Palo Alto, Redwood City, San Mateo). If you are excited about the opportunity to work with a smaller patient panel of complex care patients, contribute to a value-based care model, and work with a motivated multi-disciplinary team to provide high-quality patient care you have come to the right place. This person-centered care model works to leverage the power of our care navigators to maximize the time and energy of our providers.

What you'll do:

• Provide onsiteprimarycareservices to older adults living in Senior Living Communities
• Develop and execute detailedperson-centered careplans on allprimarycarepatients
• Work with Medical Director to identify and develop policies to address issues thatarise which affect day to daypractice, culture or clinical guidelines, including for emerging disease(s)
• Provide monthly Chronic Care Management (CCM) visits and care; review and sign off on CCM documentation
• Run regular huddles and provide leadership to the clinical team
• Identify ways of working withsenior living management andcareteams to better carry out Pine Parkcareplans and improve patient outcomes

Qualifications:

• Nurse Practitioner - preferably with geriatrics experience
• Comfortable with a fast-paced and changing environment
• Willingness to provide onsite clinical across the San Francisco Peninsula region
• Desire to work with complex care patients and support the building of a holistic care model
• Strong orientation towards collaboration and working closely with a multidisciplinary team; values input of all members of the clinical team
• Strong written and verbal communicator
• Ability to work with tech-enabled care delivery tools (e.g.,project management, 'checklist' management, remote monitoring, etc.) used together to improve patientcareand team function
• Flexible work schedule with travel within the San Francisco Peninsula area. This role involves driving between partner communities as needed

The base salary range for this role is $135,000 to $150,000 plus generous bonus, equity, and benefits. The final compensation package for each successful candidate will depend on several job-related factors unique to each candidate. These factors may include but are not limited to education, training, skill set, years and depth of experience, certifications and licensure, business needs, internal peer equity, and alignment with geographic and market data. Our compensation structures and ranges are tailored to each geographic zone's unique market conditions to ensure all employees receive fair and competitive compensation based on their roles and locations.
