Job summary

CivicMinds, Inc. is seeking a qualified Physician to provide medical care across skilled nursing and long-term care facilities in California. The role offers weekday-focused rounds, with optional after-hours call coverage for additional compensation.

The physician will oversee patient care, supervise NPs, and coordinate with the care team to ensure quality outcomes. Flexible scheduling supports work-life balance for full-time clinicians.

Qualifications

• MD/DO degree from an accredited institution.
• Board-certified medical specialty required.
• Active California medical license.
• Current DEA registration.
• Current CPR/BLS certification.
• Experience in Internal Medicine, Geriatrics or Palliative Care.

Responsibilities

• Provide physician coverage across multiple SNFs and long-term care facilities.
• Conduct patient rounds and evaluate admissions and residents.
• Diagnose and manage acute and chronic conditions.
• Develop and oversee individualized treatment plans.
• Supervise and collaborate with nurse practitioners.
• Monitor progress and coordinate discharge from skilled care.
• Participate in daily clinical discussions for ACO REACH patients.
• Respond to calls and clinical concerns during designated hours.
• Maintain accurate medical records and billing documentation.
• Coordinate transfers to hospitals when needed.
• Engage in quality improvement and medical staff meetings.
