Job summary

The Geriatrics and Extended Care Service (GEC) at the U.S. Department of Veterans Affairs is seeking a clinician to provide comprehensive geriatric care across inpatient, outpatient, and home-based settings.

Responsibilities include managing a panel of Veterans with complex conditions, developing individualized care plans, and collaborating with a multi-disciplinary team to optimize health and quality of life.

Responsibilities

• Manages a panel of veterans with acute and chronic conditions, focusing on health promotion, disease prevention, function maximization, and honoring care goals.
• Develops and implements individualized plans to achieve optimal population health and disease management; leads multidisciplinary collaboration.
• Orders and interprets diagnostic studies and communicates with patients and families; coordinates care with nursing and other disciplines.
• Provides education to patients and families; initiates referrals; assesses and adjusts the plan of care to improve outcomes; upholds ICARE values.
