Summary

The Geriatrics and Extended Care Service (GEC) provides care in several clinical venues including inpatient - outpatient - community - and research settings - including but not limited to: Community Living Center (CLC) - Hospice and Palliative Medicine (Hospice House and outpatient) - Geriatric Evaluation and Management Clinic (GEM Clinic) - Home Based Primary Care - Inpatient and Outpatient Consultation Services (Geriatric and Hospice Services) - Community Outreach - Education and Research Services.

Duties

• Duties include but are not limited to: Independently manages a panel of patients with acute and chronic health conditions with an emphasis on health promotion and disease prevention - maximization of function - and/or assessing and honoring seriously ill Veteran's goals and preferences for care and quality of life using evidence-based practice
• Develops and implements an individualized plan to achieve optimal population health and disease management - Demonstrates leadership Works collaboratively multi-disciplinary team Orders and interprets diagnostic studies Communicates effectively with patients - patient caregivers - colleagues - and interdisciplinary team members Cooperates with nursing staff and other disciplines to provide patient care - Prescribes pharmacologic and non-pharmacologic interventions
