Job summary

St. Elizabeth Physicians seeks an Experienced Geriatric APP to join PACE and oversee community care for enrollees. You will perform CMS regulatory visits with the attending physician and share clinical information with the interdisciplinary team to ensure optimal care.

Working with eight counties in Northern Kentucky, you will coordinate care with physicians, families and community partners and help develop treatment protocols in collaboration with the medical director.

Qualifications

• Must hold an active Nurse Practitioner license in Kentucky (or eligible).
• Experience in geriatric care and PACE or long-term care settings preferred.
• Ability to perform CMS face-to-face visits and coordinate care with a physician.

Responsibilities

• Oversee community care for PACE enrollees and coordinate with the interdisciplinary team.
• Complete CMS regulatory visits face-to-face with the attending physician.
• Communicate clinical information to the team to ensure appropriate care.
• Develop and review protocols for treatment with the physician.
• Coordinate care with physicians, family members and community partners as needed.
