Job summary

St. Elizabeth HealthCare is seeking a PACE Geriatric Medicine Advanced Practice RN in Fort Thomas, KY to oversee community care for PACE enrollees.

The role collaborates with the attending physician to fulfill CMS visits and provide clinical information to the interdisciplinary team for appropriate care. The candidate should have substantial geriatric experience and be able to coordinate care with physicians, families, and community partners.

Qualifications

• Knowledge of advanced practice in geriatrics including anatomy and physiology, pathophysiology, pharmacology and therapeutics.
• Knowledge of environmental-specific problems in the aging population.
• Ability to coordinate with physicians, family and community partners.

Responsibilities

• Oversees community care for PACE enrollees.
• Completes CMS regulatory visits face-to-face in collaboration with the attending physician.
• Coordinates care with the interdisciplinary team and with physicians and family.
• Reviews and develops treatment protocols in collaboration with the physician.
• Travels as needed to long-term care facilities.
