Inspire Health Alliance seeks Advanced Practice Providers (NPs/PAs) to join our San Diego PACE program, where you’ll see fewer patients per day with 45–90 minute appointments. You’ll work with an 11-discipline interdisciplinary team to deliver comprehensive geriatric care in a supportive, patient-centered setting.

Enjoy time with each participant, a streamlined care flow, and meaningful work focused on improving outcomes for seniors in the PACE program in Downtown San Diego.

Qualifications

• Active California NP or PA license with DEA
• Nation-wide board certification (AGPCNP, FNP, or PA-C) preferred; eligible in CA
• Experience or interest in geriatrics, PACE, SNF, home health, or Medicare Advantage a plus

Responsabilités

• Deliver primary care, chronic disease management, and palliative care to complex, frail seniors.
• Participate in daily morning interdisciplinary team huddles to coordinate participant care.
• Provide clinical oversight for participant panels across clinics, assisted living facilities, and post-acute settings.
• Complete thorough clinical documentation using ICD-10/HCC coding per Medicare/Medi-Cal standards
