Family Health Centers of San Diego seeks a Nurse Practitioner or Physician Assistant to join the PACE College Grove team in San Diego. The role focuses on holistic care for elderly patients within an integrated health team, in a dedicated outpatient setting.

You will care for about 12 patients daily, manage documentation, and participate in IDT meetings while maintaining licensure and meeting organizational metrics. Multicultural care is central to this position.

Qualifications

• Minimum of 2 years of experience with underserved, low‑income older adults in outpatient settings.
• Completion of an approved/accredited Physician Assistant or Nurse Practitioner program.
• Current unrestricted license to practice in California or ability to obtain.
• DEA license or ability to obtain.
• Current CPR/BLS or ACLS/ATLS or Advanced Resuscitation Training.
• Spanish language preferred due to patient population.

Responsabilités

• Provide care to roughly 12 patients per day with an integrated health team.
• Complete all required documentation per department standards.
• Engage with diverse social, cultural, and economic patient groups.
• Attend Interdisciplinary Team (IDT) meetings and participate in outcomes programs.
• Maintain certifications/licensures and adhere to organizational policies.
• Support administrative duties including lab reports, charts, and medication refills.
