Job summary

Suvida Healthcare is seeking a Neighborhood Center Advanced Practice Provider to drive, support, diagnose, and care for a range of health issues among Medicare-eligible Hispanic seniors. Under the supervision of the Neighborhood Center Medical Director, you will operate in a multidisciplinary value-based care model, handling low to high complexity cases with autonomy.

You will lead your care team, partner with clinicians across disciplines, and focus on preventive care and health maintenance

Qualifications

• Active medical license in Texas or Arizona
• Geriatrics experience preferred
• Bilingual (English/Spanish) preferred
• Proficiency with Microsoft Office applications

Responsibilities

• Provide outpatient care and preventive services.
• Manage patients under supervision in a value-based care model.
• Lead your care team within the Neighborhood Center.
• Collaborate with multidisciplinary care team.
• Maintain complete medical histories and records.
• Coordinate referrals to specialists as needed.
• Attend daily and weekly huddles to discuss patient care and close gaps.
• Influence department strategy and make independent decisions on complex issues.
