Job summary

Suvida Healthcare is seeking a Neighborhood Center Advanced Practice Provider to deliver outpatient care under the Medical Director in a value‑based care setting. You will lead your care team and coordinate with a multidisciplinary group to manage diverse patient needs, emphasizing quality, access, and prevention.

Ideal candidates hold NP or PA licensure with geriatrics board readiness, are licensed in TX or AZ, and communicate effectively in English and Spanish to serve our diverse senior

Qualifications

• At least 2 years of experience, preferably in geriatrics.
• Active, unrestricted medical license in TX or AZ.
• Board certification or eligibility in Family, Adult, and Geriatrics.
• Experience leading a care team is a plus.

Responsibilities

• Cares for patients in outpatient medicine, focusing on continuity of care and disease prevention.
• Manages patients under supervision in a multidisciplinary value‑based care model.
• Leads their care team within the Neighborhood Center and collaborates with the team.
• Focuses on high‑quality patient service and improves health outcomes.
