Job summary

Suvida Healthcare is seeking a Neighborhood Center Physician to lead outpatient geriatric care in a value-based model. The physician will drive coordination across a multidisciplinary team within the Neighborhood Center and ensure comprehensive, culturally competent care for Medicare-eligible seniors.

The role requires an MD/DO with board eligibility in Family or Internal Medicine, a current DEA license, and licensure in Texas or Arizona.

Qualifications

• 3+ years minimum experience, preferably in geriatrics.
• Active, unrestricted medical license in Texas or Arizona.
• Board certification or eligibility in Family or Internal Medicine.
• Current DEA license.

Responsibilities

• Cares for patients in outpatient medicine and manages care in a multidisciplinary value-based model.
• Leads the respective care team within the Neighborhood Center.
• Collaborates with the multidisciplinary care team to ensure high-quality care.
• Attends daily morning and weekly care team huddles to address patient needs and gaps.
• Oversees Advanced Practice Providers as needed and reports to the Neighborhood Center Medical Director.
