Job summary

Prisma Health SeniorCare PACE in Columbia, SC seeks a BC/BE Internal Medicine, Family Medicine, or Geriatrician (preferred) to join the SeniorCare PACE team as the primary care provider for frail elderly participants.

Join an interdisciplinary team and provide care across PACE centers, homes, hospitals, and skilled nursing facilities. Geriatric assessments, care coordination, and family discussions are integral, with teaching opportunities at USC School of Medicine commensurate with experience.

Qualifications

• BC/BE Internal Medicine, Family Medicine, or Geriatrician (preferred)

Responsibilities

• Serve as the primary care provider for PACE participants, conducting regular assessments and managing medical needs.
• Work within an interdisciplinary team of nurses, social workers, therapists, dietitians, and other specialists.
• Provide care in multiple settings (PACE center, participant's home, hospitals, and skilled nursing facilities).
• Perform geriatric assessments addressing cognitive impairment, functional decline, polypharmacy, and end-of-life planning.
• Participate in care coordination and family discussions to align care goals with the participant's preferences and needs.
• Collaborate with specialists and hospital teams when advanced interventions are needed.
• Teaching opportunities available with an academic appointment at the University of South Carolina School of Medicine, commensurate with experience.
