Job summary

Spectrum Retirement Communities, a provider groups serving residents in Illinois, is seeking a Wellness Nurse (LPN or RN) on a PRN schedule. You will support clinical care, coordinate with nurses and providers, and ensure regulatory compliance while delivering compassionate, resident-centered care.

Responsibilities include admission screening, care plan adherence, medication administration, and documentation of changes. State regulatory knowledge and a caring attitude are essential.

Qualifications

• Graduate from an accredited school of practical nursing.
• Current nursing license and state registration (LPN or RN).
• Three years' experience in assessment and geriatric nursing.
• Knowledge of state regulations, policies, and procedures.

Responsibilities

• Assist in coordination of the resident admission process including screening, assessment, and care needs.
• Ensure care is delivered as indicated in resident care plans.
• Medication and treatment administration and ensure medications are administered per physicians’ orders.
• Assistance with coordinating care and services with other outside providers including resident physicians.
• Document and report care provided and resident observations including unusual or significant changes.
• Maintain a working knowledge of state regulations, policies, and procedures.
