Job summary

Spectrum Retirement Communities is seeking a Wellness Nurse for PRN shifts in our Ohio community. The role focuses on ongoing clinical monitoring and supporting residents, with collaboration among nurses, caregivers, and medication personnel to uphold regulatory compliance and clinical excellence.

Responsibilities include admission coordination, care plan delivery, medication administration, and coordination with external providers.

Qualifications

• Graduate from an accredited school of practical nursing.
• Current nursing license and state registration (LPN or RN).
• Three (3) years of experience in assessment and geriatric nursing preferred.

Responsibilities

• Assist in coordination of the resident admission process including screening, assessment, and care needs.
• Ensure care is delivered as indicated in resident care plans.
• Medication and treatment administration and ensure medications are administered per physicians’ orders.
• Assist with coordinating care and services with other outside providers including resident physicians.
• Document and report care provided and resident observations including unusual or significant changes.
• Maintain a working knowledge of state regulations, policies, and procedures.
