Responsibilities

• Provide direct services to adults at the Geriatric Transition Center to support recovery and independent living skills.
• Maintain a clean, safe environment and manage household tasks and safety checks.
• Participate in team meetings and trainings as scheduled.
• Administer and document medications per physician orders and training; maintain CPR/First Aid certification.
• Intervene in crisis situations and make referrals as needed.
• Support personalized recovery planning with goal setting and documentation.
