Job summary

West Tennessee Post Acute in Jackson, TN seeks a Speech-Language Pathologist to assess and treat speech, language, and swallowing disorders in geriatric patients. You will develop individualized plans and provide therapy across articulation, language, voice, and cognitive domains.

Collaborating with a multidisciplinary team, you will monitor progress, adjust plans, and educate patients and families on home exercises.

Qualifications

• New graduates welcome; CF supervision available.
• Experience as Speech Language Pathologist (SLP) or Speech Therapist (ST) is a plus.
• Geriatric population experience is a plus.
• Active state licensure as a Speech Language Pathologist.
• CPR Certification is required.
• Certificate of Clinical Competence in Speech-Language Pathology (CCC-SLP).
• In good standing with all regulatory agencies and licensing boards is required.

Responsibilities

• Conduct comprehensive assessments to evaluate patients' speech and language abilities.
• Develop and implement individualized treatment plans targeting speech, language, swallowing goals.
• Provide interventions including articulation, language development, voice therapy, and cognitive training.
• Monitor and document progress; adjust treatment plans as needed.
• Collaborate with occupational therapists, physical therapists, and nurses to coordinate care.
• Educate patients, families, and caregivers about disorders, strategies, and home exercises.
• Participate in team meetings and case conferences regarding discharge planning.
