Qualifications

• 3–5 years of geriatric behavioral health experience in clinic/home settings or via telehealth.

• Satisfactory background checks and health clearances/immunizations as required by policy and state law.

Responsibilities

• Deliver behavioral health support through counseling, psychoeducation, and care coordination.
• Perform psychosocial assessments and cognitive screenings to support diagnosis and care planning.
• Collaborate with providers, nurses, and memory care specialists to update individualized care plans.
• Educate families on disease progression, safety planning, and caregiver support.
