Job summary

You will conduct psychosocial assessments, collaborate with families and healthcare teams, and coordinate with community resources to support residents and their goals, while documenting outcomes and shaping care plans.

Qualifications

• Associate's degree in Social Work or related field is required.
• Minimum 1+ years of experience in a social work setting.
• Strong communication, interpersonal, and problem-solving skills are essential.
• Ability to work with diverse populations and manage multiple tasks.

Responsibilities

• Provide psychosocial assessments and develop individualized care plans for residents.
• Identify and address social and emotional needs of residents, including mental health, trauma, and aging.
• Develop and maintain relationships with residents, families, and healthcare teams to ensure comprehensive care.
• Coordinate with outside agencies and community resources to access support services for residents.
• Monitor and report on resident outcomes and program effectiveness.
