Job summary

Mount Sinai Medical Center in Miami, Florida, seeks a social worker to support Eldercare enrollment, assessments, and ongoing care planning. The role collaborates with an interdisciplinary team to create comprehensive care plans for participants and to coordinate transitions to contracted facilities when needed.

The ideal candidate holds an MSW, is experienced in hospital or community settings, and possesses BLS/CPR certification.

Qualifications

• Masters in Social Work required from an accredited program.
• Two years of social work experience in hospital, nursing home, or community setting desired; prior frail/elderly population experience preferred.
• BLS/CPR certification required.

Responsibilities

• Perform in-person initial assessments for eldercare enrollment to gather psycho-social history and needs.
• Collaborate with Interdisciplinary Team to develop and update care plans for participants.
• Conduct semi-annual reassessments and when conditions change.
• Attend daily IDT meetings and coordinate 24-hour care planning with team members.
