Job summary

Habitat Health is seeking a dynamic Social Worker to support elderly participants within the PACE program in Sacramento. You will work under the Social Services Manager as part of the IDT, conducting assessments and implementing care plans to improve health outcomes and quality of life.

Responsibilities include care coordination, counseling, and resource facilitation, with collaboration across the care team to ensure seamless transitions between settings.

Qualifications

• MSW required with clinical social work background.
• Experience coordinating and facilitating care conferences.
• CPR/BLS certification current or within 30 days of hire.
• TB test and immunizations up to date as required.

Responsibilities

• Conduct social work assessments to determine psychosocial needs, preferences and goals.
• Deliver and document social work interventions per care plan, arrange resources and services.
• Collaborate with primary care and care team to ensure smooth transitions of care.
• Coordinate care conferences and participate in IDT meetings for care optimization.
• Provide discharge planning when participants disenroll from the program.
