Baptist Health Princeton Hospital in Birmingham hires a Social Worker LMSW for full-time days in the Geriatric Psychiatry unit. You will assess resident needs, plan care, and coordinate discharge to ensure the highest practicable wellbeing.

Emphasis on family education, consent, and rights, with multidisciplinary collaboration. Candidate should have a Bachelor’s in social work, and a Master’s in Social Work is highly preferred; LMSW preferred.

Responsibilities

• Develops preliminary and comprehensive assessments, written plans of care, discharge plans and progress notes.
• Provides residents and families with information concerning rights and advance directives.
• Collaborates with residents, families, physicians, and staff to support wellbeing and problem solve.
