Baptist Health Princeton Hospital in Birmingham is seeking a Social Worker LMSW for PRN Day shifts in Case Management. The role supports residents’ emotional and social well-being, coordinating care and discharge planning in a hospital setting.

Requirements include a Bachelor's in social work or related field, with Master’s and LMSW preferred. One year of supervised health care social work experience is expected. Benefits are included starting day one.

Qualifikationen

• Bachelor’s degree in social work or a Bachelor’s degree in a human services field is required.
• Master’s in Social Work HIGHLY preferred.
• LMSW HIGHLY preferred.
• One (1) year supervised social work experience in a health care setting working directly with individuals.

Aufgaben

• Develops preliminary and comprehensive assessments, written plans of care, discharge plans and progress notes; reviews and revises these plans as necessary.
• Provides residents, family and/or resident representatives with information concerning residents’ rights and advance directives.
• Develops an effective working relationship with resident, family, team members, physicians, community and others to advocate and problem solve on behalf of the resident.
• Ensures residents have access to prescriptions, durable medical equipment (DME), and other services as identified upon discharge.
• Refer residents to education regarding healthcare delivery and reimbursement systems, prescription drug programs, health & wellness programs, community agencies, housing options, and other services.
• Participates in committees and Performance Improvement Projects.
• Maintains reasonably regular, punctual attendance consistent with Baptist Health policies and applicable laws.
