Job summary

Baptist Health Princeton Hospital is seeking a Social Worker LMSW to serve residents and families, addressing emotional and social needs to support well-being. The role emphasizes collaboration with interdisciplinary teams and smooth discharge planning, with an expected schedule of 8:30 am to 5 pm and a minimum of four shifts monthly, including weekends.

Qualifications include a Bachelor's in social work or related field and LMSW preferred.

Qualifications

• 1 year supervised social work experience in a health care setting working directly with individuals.
• Bachelor’s degree in social work or a related field required.
• Master’s in Social Work strongly preferred.
• LMSW preferred.

Responsibilities

• Develop preliminary and comprehensive assessments, care plans, discharge plans, and progress notes, updating as needed.
• Provide residents, families and representatives with information on rights and advance directives.
• Collaborate with the resident, family, team, physicians, and community to support well-being.
• Ensure access to prescriptions, DME, and other services at discharge.
• Refer residents to education on healthcare delivery, reimbursement systems, and community resources.
• Participate in committees and Performance Improvement Projects.
• Maintain regular, punctual attendance and compliance with policies and standards.
