Job summary

Mount Sinai Medical Center is seeking an experienced Social Worker for the Eldercare program to assess patients, develop care plans, and coordinate with a multidisciplinary team. You will support families, facilitate admissions and discharges, and advocate for patient needs across the continuum of care.

Responsibilities include regular reassessment, end-of-life planning, and linkage to community resources, with a focus on compassionate, holistic care for the elderly and their caregivers.

Qualifications

• Masters in Social Work from an accredited school.
• BLS or CPR certification required.
• Two years of social work in hospital/nursing home/community setting preferred.

Responsibilities

• Perform initial psychosocial assessments for Eldercare enrollment and document cognitive status and supports.
• Collaborate with the interdisciplinary team to develop care plans for participants.
• Conduct six‑monthly reassessments and communicate changes in care.
• Attend IDT meetings and coordinate with caregivers and community agencies.
• Provide ongoing support, counseling, and education to participants and families.
• Facilitate admissions/discharges to contracted facilities when needed.
• Advocate for participants and assist with advance directives and benefits.
• Support end-of-life planning and connect to community resources.
