Elm Terrace Gardens in Lansdale, PA is seeking a compassionate Social Worker to support residents and families in a skilled nursing setting. The role focuses on counseling, resource coordination, and facilitating transitions to ensure quality of life.

You will develop care plans, collaborate with multidisciplinary teams, document interactions in accordance with regulations, and advocate for residents’ rights. Full-time exempt with strong benefits after 60 days and several paid time off perks.

Qualifications

• Bachelor’s degree in Social Work (BSW) or related field; Master’s degree preferred but not required.
• Valid state licensure or certification as a Licensed Social Worker (LSW) or equivalent.
• Proven experience working in healthcare or skilled nursing environment.
• Strong interpersonal and communication skills, with the ability to build rapport with diverse populations.
• Knowledge of healthcare regulations, Medicaid/Medicare policies, and community resources.
• Ability to handle sensitive situations with empathy, professionalism, and discretion.
• Excellent organizational and documentation skills.
• Ability to work collaboratively within a multidisciplinary team.

Responsibilities

• Conduct psychosocial assessments of residents to identify needs and barriers to care.
• Develop and implement individualized care plans with residents, families, and healthcare teams.
• Provide emotional support to residents and families to cope with health challenges and transitions.
• Coordinate with community resources, agencies, and support services to meet residents’ social and emotional needs.
• Advocate for residents’ rights and preferences within the healthcare setting.
• Document interactions, assessments, and care plans accurately and in compliance with regulatory standards.
• Participate in interdisciplinary team meetings to ensure comprehensive resident care.
• Educate residents and families about available services, benefits, and community resources.
