Job summary

Oakridge Manor in Ferndale, MI is seeking a dedicated Social Worker to identify residents' social, emotional, and psychological needs and develop personalized plans to help them reach their full potential during their stay.

You will educate residents and families about post-acute options, coordinate care with the interdisciplinary team, and manage discharge planning, care conferences, and documentation in the EMR.

Qualifications

• Bachelor's degree in social work or related human services field.
• At least 1 year of supervised social work experience in a health care setting.

Responsibilities

• Complete and document Social Work assessments within 3 days of admission.
• Ensure 3877/78 forms are completed and accurate, upon admission and annually.
• Coordinate care with the IDT and conduct Resident Care Conferences.
• Develop and update Care Plans including Psychosocial, Discharge Planning, Behavioral, Cognitive, mood, and related needs.
• Educate residents and families about Advanced Directives and related processes.
• Document all interactions with residents, families, and health care providers in the Medical Record.
• Demonstrate proficiency with EMR for the facility.
• Involve in discharge planning and coordination of DME & Home Care as needed.
