Qualifications

• Bachelor’s degree in Social Work, Human Services, Psychology, Sociology, or related field is required; Master’s degree preferred.

Responsibilities

• Advocate for residents' psychosocial, emotional, and social needs.
• Complete social services assessments and participate in development/review of care plans.
• Meet with residents and families to identify concerns and provide support.
• Coordinate and participate in care conferences and interdisciplinary team meetings.
• Assist residents and families with discharge planning and coordination of services.
• Communicate with hospitals, physicians, and community agencies as needed.
• Help residents understand resources, benefits, and community programs.
• Maintain timely documentation in accordance with policies and regulations.
• Assist with advance directives, guardianship, and power of attorney matters.
• Provide emotional support during illness, transition, loss, and adjustment.
