Job summary

Pine Creek in Wayne, MI is seeking a Part-Time Social Worker to identify residents' social, emotional and psychological needs and to develop plans to reach their full potential during their stay.

You will educate patients and families about post-acute options, assist with discharge planning, coordinate with the care team, and ensure accurate documentation of assessments and care plans.

Qualifications

• Bachelor's degree in social work or a human services field is required.
• Minimum 1 year of supervised social work experience in healthcare.

Responsibilities

• Complete and document social work assessments within 3 days of admission.
• Ensure 3877/78 forms are completed and accurate on admission and annually.
• Schedule and document 72-hour Resident Care Conference and notify IDT.
• Coordinate care with psych provider for medications and consents.
• Develop and update Care Plans including psychosocial, discharge planning, and cognitive needs.
• Educate residents and families on advance directives and discharge rights.
