Job summary

Eastside Center for Health & Rehabilitation in Bangor, ME seeks a Social Worker to advocate for residents’ well-being and ensure holistic needs are met. You will identify social, emotional, and environmental needs and collaborate on individualized care plans to enhance quality of life.

The role includes counseling residents and families, supporting transitions, and linking to community resources. Experience in healthcare or geriatric settings is preferred.

Qualifications

• Bachelor's degree in Social Work or a related field.
• Ability to communicate both verbally and in writing to convey assessments and findings.
• Ability to assess social, emotional and psychological needs and develop treatment plans.
• Ability to work with supervisors, coworkers and facility staff.
• Ability to work independently and creatively.
• Ability to work hours as scheduled.
• Experience in health care and/or geriatric setting.
• Knowledge of state and federal regulations governing social services in a nursing facility.

Responsibilities

• Conduct comprehensive psychosocial assessments to identify residents' social, emotional, and environmental needs, as well as strengths and preferences.
• Collaborate with residents, families, and the interdisciplinary team to develop individualized care plans.
• Provide counseling and emotional support to residents and families, guiding coping strategies and adjustment to care transitions.
• Advocate for residents' rights and preferences in care planning and decision-making.
• Connect residents and families with community resources, support services, and advocacy organizations.
• Provide crisis intervention and support during challenging situations.
