Job summary

National Health Care Associates is seeking a Social Worker for our Eastside location in Bangor, ME. You will advocate for residents’ well-being, coordinate with families and an interdisciplinary team, and support residents through transitions and challenges.

You will assess psychosocial needs, develop individualized care plans, and provide counseling while connecting families with community resources. Experience in geriatric care is valued.

Qualifications

• Bachelor's Degree in Social Work or a related field.
• Ability to communicate both verbally and in writing to convey assessments and findings.
• Ability to assess residents' social, emotional and psychological needs and develop treatment plans.
• Ability to work with supervisors, colleagues and facility staff in performance of duties.
• Ability to work independently and creatively.
• Ability to work hours as scheduled based on position requirements.
• Experience in health care and/or geriatric settings.
• Knowledge of state and federal regulations governing social services in a nursing facility.

Rate of Pay

USD $20.00 - USD $25.00 /Hr.

What You'll Bring:
