Job summary

Elderwood at Burlington is seeking a Social Worker to support residents and families through transitions, helping ensure dignity, emotional well-being, and connection throughout the care journey.

You will advocate for resident rights, collaborate with the care team, and provide counseling and referrals to enhance quality of life for older adults in our skilled nursing and rehabilitative setting.

Qualifications

• Bachelor’s degree in Social Work or related field.
• At least one year of supervised social work experience in healthcare.
• Strong communication, documentation, and collaboration skills.
• Genuine interest in working with older adults and healthcare transitions.

Responsibilities

• Welcome and support residents and families during admission.
• Assess psychosocial, emotional, and support needs of residents.
• Participate in care planning and interdisciplinary team meetings.
• Assist with discharge planning and transitions of care.
• Complete required documentation including admission and discharge forms.
