Qualifications

• B.A. in Psychology or Sociology or a B.A./M.A. in Social Work or LCSW certificate.
• Two years of experience in social work in a long-term care environment required.

Responsibilities

• Greet residents and orient them to the building and staff.
• Maintain and complete social service admission notes, social history, social assessment, and discharge planning.
• Write quarterly and annual updates of Social Assessment and Social History.
• Record significant events in residents' life and social service contacts.
• Monitor and assist psychosocial staff and supervise psychosocial aides.
• Inform residents and families about prepaid burial/funeral arrangements and follow up.
• Complete the Social Service section of the MDS form on admission, quarterly, annually, and for significant changes.
• Maintain personal contact with residents and families to identify changing conditions and needs.
• Coordinate room changes and notify involved departments; document changes in medical records per IDPH regulations.
• Address resident and family problems and reconcile differences when possible.
• Provide information on Advanced Directives and assist in completing documents; obtain physician acknowledgments for surrogate forms; maintain DNR forms in database.
• Contact families of private-pay residents after discharge to determine bed hold options and report to billing.
