Job summary

Qualifications

Responsibilities

• Greet and orient new residents to the facility.
• Complete social service assessments and discharge planning when appropriate.
• Maintain social service documentation including annual and quarterly updates.
• Coordinate discharge planning with families and outside programs.
• Provide information to residents and families about Advanced Directives and surrogate forms.
• Perform other related duties as directed by the Administrator.
