Job summary

Touchmark at Meadow Lake Village in Idaho is seeking a Resident Care Manager to enrich residents' lives by ensuring dignity and high-quality care. The role requires coordinating care plans, communicating with health professionals, and guiding a caregiving team on-site.

The ideal candidate will have two years in licensed care, caregiver and med tech experience, and a med aide card with CPR/First Aid certification.

Qualifications

• Minimum of two years of successful experience in licensed care.
• Experience as a caregiver and med tech is required.
• Basic understanding of the conditions and feelings of older adults.
• Willingness to continue learning and improving skills; confidentiality commitment.
• Med aide card is required and CPR/First Aid certification if needed.

Responsibilities

• Serve as the primary contact for residents or families regarding care, scheduling, and care plan reminders.
• Communicate with physicians and health-care professionals, discharge planners, and family members.
• Observe residents, report observations, and support care plan changes with documentation.
• Assist with recruitment and onboarding of new caregiving team members.
• Assist with scheduling of caregivers.
• Complete quality assurance audits and maintain organized resident charts.
• Provide on-site care to residents during outbreaks and pandemics.
