Job summary

Sweetwater Care is seeking a licensed social worker to assess and support residents' psychosocial and emotional needs in a skilled nursing setting. You will document and coordinate care plans in alignment with NASW ethics and regulatory requirements.

Responsibilities include comprehensive assessments, interdisciplinary care planning, discharge planning, and resident advocacy. A Bachelor’s in Social Work or Human Services and state licensure are required; geriatric experience is valued.

Qualifications

• Bachelor's Degree in Social Work or a Bachelor's Degree in Human Services field from an accredited school of social work is required.
• Certified, licensed, or registered in the state of practice is required.

Responsibilities

• Completes social history and psychosocial assessment for each resident identifying social, emotional, and psychological needs; uses MDS data to guide care planning.
• Maintains accurate documentation compliant with federal/state regulations and center policies (Advanced Directives, MDS, Care Plans, progress notes, etc.).
• Supports resident's right to self-determination; informs residents of therapeutic alternatives and educates on options when services are refused.
• Participates in developing an interdisciplinary care plan addressing psychosocial needs and goals with appropriate social worker interventions.
• Provides therapeutic support to aid residents’ coping with transition to long-term care and adjust psychosocially.
• Provides education to residents and families to assist understanding of placement and facility issues and refers to social service agencies as needed.
• Coordinates discharge planning and makes referrals for home care services prior to return to community.
• Works with the interdisciplinary team to promote resident rights and psychological well-being; addresses abuse as mandated.
• Assists residents, families, and staff in proactive self-advocacy to improve quality of life and care.
• Assists in planning, organizing, and directing social service programs; participates in inspections and corrective actions as needed.
• Other duties may be assigned.

Education

Bachelor's degree in Social Work or Human Services
