American Senior Communities is hiring a Social Services Assistant to deliver medically related social services under the Social Services Director. This on-site, full-time role supports the well-being of residents in our Indiana communities.

Responsibilities focus on applying social work principles, coordinating with interdisciplinary teams, and fostering a compassionate environment for residents and families. CNA preferred.

Qualifikationen

• Understanding social work principles, psychosocial assessments and interventions.
• Empathetic engagement with residents and families, and clear communication with interdisciplinary teams.
• Creating a comforting and engaging atmosphere for residents.
• Works with and promotes collaboration among departments, professionals, consultants, and organizations including government regulatory agencies.
• The ability to work towards a common goal of excellent care for our residents.
